les patients ne confient pas facilement cette reacutegion agrave un

theacuterapeute mecircme en labsence de douleur

Glisser un coussin sous les genoux du patient afin de ne

pas entraicircner dinconfort lombaire

Ne pas heacutesiter agrave recouvrir le buste de la patiente dun

pull afin de preacuteserver sa pudeur et deacuteviter une

sensation de refroidissement freacutequente lors de ce type

de traitement Laisser la tecircte du patient sur la table vous

permettra dinterrompre la seacuteance (teacuteleacutephone) en conservant le relacircchement obtenu

Jugeacutee utile pour diminuer les douleurs lorsquun reacutetreacutecissement du trou de conjugaison entraicircne une compression

radiculaire pour diminuer la pression exerceacutee sur les capsules des articulaires posteacuterieures la traction cervicale a

fait lrsquoobjet drsquoun consensus professionnel [1] mais des meacuteta-analyses anciennes ne se prononccedilaient cependant pas

clairement en faveur ou en deacutefaveur de cette prise en charge manuelle ou instrumentale du fait de nombreux biais

meacutethodologiques [48 33]

Validiteacute

Lrsquointensiteacute de la traction neacutecessaire pour observer un effet physique serait de lrsquoordre de 12 kgs [10]

une traction de lrsquoordre de 5 kg nrsquoayant pas drsquoeffet significatif sur lrsquoespace interverteacutebral [26]

Elle nrsquoaltegravererait pas le flux arteacuteriel verteacutebral de sujets sains avec ou sans rotation [44]

kineacutesitheacuterapeute assis en bout de table

Les mains ouvertes paumes vers le

haut sont placeacutees lrsquoune sur lrsquoautre sous

le cou du patient Installer les mains confortablement

afin de pouvoir reacutealiser longtemps la manœuvre

Tracter doucement en direction cracircniale et maintenir la

traction Le relacircchement se manifeste par une deacutecompression unilateacuterale alterneacutee agrave lrsquoimage drsquoun tiroir coinceacute qui

srsquoouvre gracircce agrave des mouvements lateacuteraux

le haut les doigts en flexion meacutetacarpienne et en extension des IPP et IPD en contact avec la zone sous-occipitale

La force correctrice est constitueacutee par le poids de la tecircte du patient qui vient reposer doucement dans les paumes

selon un angle de 15deg de flexion

pouvant ecirctre modifieacute en fonction du

confort du patient

Des tractions intermittentes de 60 secondes sont entrecoupeacutees de 20 secondes de relaxation

Une force initiale de 55 kg est imposeacutee ajusteacutee en fonction de la toleacuterance du patient et de la reacuteponse des

symptocircmes douloureux

de la toleacuterance et de lrsquoabsence de douleurs ressenties par le patient elle

est augmenteacutee jusqursquoagrave un maximum de 9 kg (maximum de litres drsquoeau

que supporte le dispositif)

Posologie

15 minutes par jour quotidiennement sur un mois Le patient reste assis 2 minutes apregraves traitement

Jugeacutee utile pour diminuer les douleurs lorsquun reacutetreacutecissement du trou de conjugaison entraicircne une compression

radiculaire pour diminuer la pression exerceacutee sur les capsules des articulaires posteacuterieures la traction cervicale a

fait lrsquoobjet drsquoun consensus professionnel [1] mais des meacuteta-analyses anciennes ne se prononccedilaient cependant pas

clairement en faveur ou en deacutefaveur de cette prise en charge manuelle ou instrumentale du fait de nombreux biais

meacutethodologiques [48 33]

Validiteacute

Lrsquointensiteacute de la traction neacutecessaire pour observer un effet physique serait de lrsquoordre de 12 kgs [10]

une traction de lrsquoordre de 5 kg nrsquoayant pas drsquoeffet significatif sur lrsquoespace interverteacutebral [26]

Elle nrsquoaltegravererait pas le flux arteacuteriel verteacutebral de sujets sains avec ou sans rotation [44]

Patient en deacutecubitus la tecircte sur la table

kineacutesitheacuterapeute assis en bout de table

Les mains ouvertes paumes vers le

haut sont placeacutees lrsquoune sur lrsquoautre sous

le cou du patient Installer les mains confortablement

afin de pouvoir reacutealiser longtemps la manœuvre

Tracter doucement en direction cracircniale et maintenir la

traction Le relacircchement se manifeste par une deacutecompression unilateacuterale alterneacutee agrave lrsquoimage drsquoun tiroir coinceacute qui

srsquoouvre gracircce agrave des mouvements lateacuteraux

le haut les doigts en flexion meacutetacarpienne et en extension des IPP et IPD en contact avec la zone sous-occipitale

La force correctrice est constitueacutee par le poids de la tecircte du patient qui vient reposer doucement dans les paumes

traction agrave la tecircte du patient se fait

selon un angle de 15deg de flexion

pouvant ecirctre modifieacute en fonction du

confort du patient

Saunders 3D ActiveTrac reg

Des tractions intermittentes de 60 secondes sont entrecoupeacutees de 20 secondes de relaxation

Une force initiale de 55 kg est imposeacutee ajusteacutee en fonction de la toleacuterance du patient et de la reacuteponse des

symptocircmes douloureux

La centralisation et la reacuteduction maximales des symptocircmes sont rechercheacutees

La force imposeacutee lors de la phase de relaxation est de 50 de la force de traction

Chaque seacuteance de traction dure 15 minutes le patient restant au repos 2 minutes en deacutecubitus apregraves la seacuteance Elle

peut ecirctre reacutealiseacutee avant ou apregraves les exercices

Posologie

10 seacuteances reacuteparties sur 4 semaines de traitement agrave raison de 3 seacuteances par semaine les premiers 15 jours 2

seacuteances par semaine les 15 derniers jours Chaque seacuteance dure au total 30 agrave 45 minutes

Valeurs

Sur des patients drsquoacircge moyen lrsquoajout de tractions cervicales instrumentales permet agrave 6 mois une

diffeacuterence de 13 IC95 [6 - 21] points au Neck Disability Index comparativement agrave un traitement par

exercices isoleacutes (renforcement isomeacutetrique des extenseurs fleacutechisseurs du cou muscles scapulaires) Elle reste

significative agrave 12 mois [16]

Proceacutedure

Le patient chez lui dispose le Chattanooga Overdoor Traction Device

(DJO LLC) au faicirct drsquoune porte Il est assis immeacutediatement en dessous

du dispositif Une traction initiale de 3 agrave 5 kg est appliqueacutee En fonction

de la toleacuterance et de lrsquoabsence de douleurs ressenties par le patient elle

est augmenteacutee jusqursquoagrave un maximum de 9 kg (maximum de litres drsquoeau

que supporte le dispositif)

Posologie

15 minutes par jour quotidiennement sur un mois Le patient reste assis 2 minutes apregraves traitement

du dispositif Une traction initiale de 3 agrave 5 kg est appliqueacutee En fonction

de la toleacuterance et de lrsquoabsence de douleurs ressenties par le patient elle

est augmenteacutee jusqursquoagrave un maximum de 9 kg (maximum de litres drsquoeau

que supporte le dispositif)

Posologie

Le patient chez lui dispose le Chattanooga Overdoor Traction Device

(DJO LLC) au faicirct drsquoune porte Il est assis immeacutediatement en dessous

du dispositif Une traction initiale de 3 agrave 5 kg est appliqueacutee En fonction

de la toleacuterance et de lrsquoabsence de douleurs ressenties par le patient elle

est augmenteacutee jusqursquoagrave un maximum de 9 kg (maximum de litres drsquoeau

que supporte le dispositif)

Posologie

15 minutes par jour quotidiennement sur un mois Le patient reste assis 2 minutes apregraves traitement

traction agrave la tecircte du patient se fait

selon un angle de 15deg de flexion

pouvant ecirctre modifieacute en fonction du

confort du patient

Des tractions intermittentes de 60 secondes sont entrecoupeacutees de 20 secondes de relaxation

Une force initiale de 55 kg est imposeacutee ajusteacutee en fonction de la toleacuterance du patient et de la reacuteponse des

symptocircmes douloureux
