Transcript of Fréquence et impact de l'IHC post-opératoire dans les hépatectomies - Dr Nicolas Golse
Frequence et impact de l’insuffisance hepato-cellulaire
post-hepatectomie. Qu’appelle-t-on « petit foie restant » ?
Dr Nicolas GOLSEPr Eric VIBERT
PHLFGénéralités
• Mortalité < 100% – Mais 1ère cause de mortalité post-Hx– Pouvant survenir après J30
• Rarement isolée– Sepsis– Autre(s) défaillance d’organe(s)
• Multifactoriel– Chirurgicale: volume, vasculaire in/outflow, fistule…– Non chirurgicale: tox médicamenteuse, virus…
Complications correlees à l’etendue de la resection
Jarnagin WR et al. Ann Surg. 2002

PHLF: definition(s)International Study Group of Liver Surgery
• « PHLF is a postoperatively acquired deterio-ration in the ability of the liver to maintain its synthetic, excretory, and detoxifying functions, which are characterized by an increased INR and hyper-bilirubinemia on or after POD 5
• According to the normal range of […] the local laboratory
• If preoperatively increased INR or bilirubine, PHLF is defined by an increasing values on or after POD 5 compared with the values of the previous day »

« 50-50 à J5 »
Validé / foie sain ou pathologiquePrédictif de mortalité ≤ J60

Petit foie restant – quelles limites ?

Total Liver Volume ?
Facile mais source d’erreurs:
• Soustraction des tumeurs ?• PVE
• Dilatation VBIH

20% ?
• N=301• Lésions primitives ou secondaires• Standardized FLR (sFLR) = FLR volume
(mL)/SLV (mL) x 100 (%). • SLV = -794.41 + 1267.28 x body surface area
Kishi Y et al. Ann Surg. 2009
Tous types de foies inclus
Formule « difficile »
sTLV: variable selon origine géographique ?
« performing PVE in patients with sFLR 20% was not associated with improved outcome. In our practice, we perform PVE with 30% sFLR in patients who have pre-existing hepatic
injury or in patients who have received extensive chemotherapy »
• N=31• IHPH + décès: 6.5%• Morbi-mortalité à 3 mois – majorée si RLV-BWR ≤ 0.5%– RLV-TLV: cutoff 20 ou 25%
• RLV-BWR : NON corrélé avec IHPH
Truant S et al. JACS. 2007
Truant S et al. WJS. 2015
Anticipated RLVBWR ≤ 0.5% identified accurately
patients at considerable risk for death from PHLF
and comparison with the sTLV method showed a
subset of patients in whom the sTLV20%
underestimated that risk.

Take Home Messages
possible. Cut-off ?
