Compliance with Government Policy This catalog brings together for the information and convenienceof both students and staff the academic regulations of the univer-sity. University policies and regulations are subject to continu-ous review by a variety of agencies and appropriate administra-tive and governance bodies of UW-Parkside. Thus, the provi-sions of this catalog are not an irrevocable contract between thestudents and UW-Parkside. The university reserves the right tochange any provision or requirement at any time during the stu-dent’s attendance. The university reserves the right to ask thestudent to withdraw for cause at any time.
The university provides additional information and policies inseparate brochures and in the Student Guidebook. To implementthe University of Wisconsin System student disciplinary proce-dures and the academic misconduct regulations (Chapters 17and 14 of the Wisconsin Administrative Code), UW-Parksidehas developed a set of procedures which is published in theStudent Guidebook. Additional information is available on theuniversity’s website or may be secured from the Dean ofStudents, Wyllie Hall 340.
UW-Parkside does not discriminate on the basis of sex in theeducational programs or activities it operates. Thisstatement is published, in part, to fulfill the requirements of TitleIX of the Higher Education Amendments of 1972 and Section86.9 of Title 45 of the Code of Federal Regulations, the admin-istrative regulations passed pursuant to Title IX. Information required under the Higher Education Act of 1965,as amended, Title IV parts 668, 674, 675, 676, 682, 690, and692 regarding, but not limited to, eligibility and criteria forreceiving and availability of financial aid, rights and responsibil-ities of those receiving financial aid, standards of progress, termsof payment, repayment schedules for those receiving financialaid, conditions of employment related to financial aid, total edu-cational costs of attending UW-Parkside and refund policies canbe obtained from the Office of Student Financial Aid.
UW-Parkside veteran enrollment requirements and standards ofprogress information are available at the Office of the Registrar.
UW-Parkside is in compliance with the Family EducationalRights and Privacy Act of 1974 (as amended) in regards to the
right of students or their parents to review of the student’s edu-cation record, correction of information from those records andlimitation of disclosure of information contained in thoserecords. Further details are published in the UW-Parkside courseschedules. Forms authorizing institutional withholding of stu-dent records are available in the Office of Student Records.
The university is required to provide annually to every student andemployee information concerning the university’s policies on illicitdrugs and alcohol. In compliance with the Drug-Free Schools andCommunities Act amendments of 1989, UW-Parkside publishes anddistributes this information annually. Additional information is avail-able on the university’s website or may be secured from the Deanof Students, Wyllie Hall 340.
In compliance with the Immigration and Nationality Act (P.L.87-195), UW-Parkside publishes the following statement: “Thisschool is authorized under federal law to enroll nonimmigrantalien students.”
The University of Wisconsin System will provide and maintainadequate facilities for a safe and healthy learning environment. It isthe university’s responsibility to work with faculty and staff so thatthey are equipped to educate their students on practices and proce-dures that ensure safety for all members of the university.Employees with instructional responsibilities are expected to com-ply with state and federal safety laws and regulations in their insti-tutional areas. Certain courses and research projects require that thestudent work with hazardous materials while engaging in academicstudies. Instructors of these courses and research projects shallinform and train students on procedures that will maintain the stu-dents’ personal health and safety and provide them with informa-tion on the hazards of specific chemicals that will be used duringtheir course of study. Furthermore, instructors will enforce and fol-low safety policies. Prior to use of hazardous materials and equip-ment, the student shall review the procedures and information anddiscuss any associated concerns with the instructor.
The university is required through state statute (Assembly Bill 431)to share statistics with current students concerning crimes oncampus and reported campus incidents of sexual assault and daterape. As required by law, this information is distributed duringsummer orientation. In addition, federal laws, Students Right toKnow Law and the “Campus Security Act” requires the universityto inform prospective as well as current students of similar crimestatistics and information on student graduation rates. Thisinformation is distributed annually to students through the UW System Introduction and UW-Parkside’s website.
Federal law allows the University of Wisconsin system to requestand use your social security number (20 U.S.C., section 1232g).Disclosure of your social security number is voluntary.Applications received without it will be processed, and anotherform of student identifier number will be assigned. However,failure to provide the social security number may result in denialof services or benefits.
