Quand avez-vous entendu parler pour la première fois de l’Advatx?Il y a 4 ans ! Je connais un médecin de Beverley Hills qui a toujours été un partisan de la lumière jaune. Il existe 2 catégories de médecins : ceux qui aident à la validation clinique des dispositifs et ceux qui aident réellement à les concevoir. Il fait partie de cette seconde catégorie en m’expliquant il y a plus de 4 ans que de nouvelles techno-logies à base de laser jaune deviendraient importantes. Mais les appareils, à l’époque, n’étaient pas encore com-plètement aboutis. Ils étaient instables et mal pensés en termes de protocoles de traitement. Malgré tout ce médecin obtenait d’excellents résultats, et cela a piqué ma curiosité.

When did you fi rst hear about the Advatx?I’ve actually been hearing about the machine for the past 4 years. I know a Physician out of Beverley Hills who’s always been an advocate of yellow light. As you know, there are 2 types of phy-sicians: the ones that help with clinical validation of devices and the ones that actually help design them. He is the latter type. He tipped me up that solid-state yellow light was going to be signi-fi cant about 4 years ago. The problem was that systems, at that time, were somewhat inconsistent. They were somewhat uns-table and not well thought out in terms of treatment protocols. But he was getting great results, and that piqued my interest.

What do you like about it?My practice is based in the south west of the United States, in Phoenix. Before I go into this, let me explain our local demo-graphics: there are a lot of people with light skin moving to this

Qu’est-ce que vous aimez à ce sujet ?Je suis installé dans le sud-ouest des États-Unis, à Phoenix et il est important de comprendre quelle est notre démographie : une forte population à la peau claire qui se déplace dans cette partie du pays. Le comté de Maricopa, où j’habite, est le comté qui connaît la croissance la plus rapide aux États-Unis, avec environ 250 000 personnes qui s’y installent chaque année. Beaucoup de ces personnes viennent de climats plus froids et ont des phototypes de peau plus clairs allant de I à III. Inverse-ment, il existe également une population avec des phototypes IV-VI. Dès lors que nous traitons de nombreux troubles vascu-laires dus aux phototypes légers sous ce climat ensoleillé, nous devons également faire face à des problèmes de pigmentation sur des phototypes plus sombres.Le mélasma est un trouble pigmentaire qui peut avoir une com-posante vasculaire sous-jacente d’où la difficulté de choisir le bon traitement. La plupart des technologies peuvent traiter les pigments ou les vaisseaux vasculaires, mais pas les deux en même temps. C’est le cas avec des technologies comme IPL, BBL, V-Beam qui peuvent dans certains cas être efficaces mais la marge de manœuvre reste étroite. C’est pour cela que l’Advatx est bien adapté à notre climat car son profil de photo-coagulation permet une dissipation immédiate de la composante vasculaire du mélasma dans la population de peau claire tout en ciblant simultanément l’as-pect pigmentaire du mélasma. Chez les patients à peau foncée, l’Advatx apporte une sécurité inégalée. C’est extrêmement important pour moi car ici les gens s’ex-posent immédiatement au soleil après leur traitement et n’uti-lisent pas forcément une protection solaire adéquate.J’utilise la longueur d’onde de 1319 nm (en combinaison avec 589 nm) pour traiter l’acné inflammatoire, les cicatrices (y compris les cicatrices d’acné) et le raffermissement de la peau. C’est une bonne procédure pour le raffermissement de la peau. J’aime utiliser cette technique pour préparer la peau, puis la combiner avec d’autres technologies, comme la Radiofré-quence bipolaire.

Quand je teste les appareils, j’essaie de bien appréhender les avantages et les inconvénients : Que va faire cette machine pour moi ? Est-elle sûre ? Et en effet, l’Advatx est l’une des machines les plus sûres que j’ai jamais utilisées pour ces indi-cations. Il a une marge de sécurité élevée et ses protocoles offrent à une population diversifiée des traitements réussis.

Avez-vous des idées pour faire progresser cette technologie ?L’Advatx a été bien conçu avec ce gage de sécurité et peu de choses seraient à modifier. Elle se combine également très bien avec d’autres thérapies non basées sur la lumière : une solution « tout-en-un » serait excellente pour proposer une procédure complète permettant de lutter contre le vieillissement cutané.

Many people who move hear from colder cli-mates have lighter skin phototypes ranging I-III. On the other end of the spectrum there is also a diverse population with pho-totypes IV-VI . So, while we deal with a lot of vascular disorders from light phototypes in this sunny climate while we also have to deal with pigmentation issues on the darker phototypes. Melasma is a pigmentary disorder that can have an underlying vascular component. So, unless you’re using the right technolo-gy it’s a case of being doomed if you do and doomed if you don’t. By that I mean, most technologies can either treat pigment or vascular, but not at the same time. When you’re looking at IPL, BBL, V-Beam and so forth, their path to clinical success may be episodic in this climate as the the margin for success can be tight and the risk of misapplication high.The Advatx is perfect for our climate because its photocoagu-lation profile allows for an immediate dissipation of the vas-cular component of melasma in the fair skin population while simultaneously effectively targeting the pigmentary aspect of melasma. In the darker-skinned patients, the Advatx brings an unparalleled safety profile. This is hugely important to me. Remember: people here go out in the sun immediately after a treatment and they don’t always wear adequate sun protection. I use the 1319nm wavelength (in combination with 589nm) to treat inflammatory acne, scars (including acne scars), and skin tightening. And it is a good foundation for any skin tightening endeavours. I like to use it first to prep the skin and then com-bine it with other technologies, such as bipolar RF.

When I look at devices I try to figure out cost-benefit analysis: What is this machine going to do for me? And, as the number of non-specialists join the fight, safety becomes increasingly paramount. Because of the issues associated with older tech-nologies, the medical community has placed a higher burden on safety.The Advatx is one of the safest machines I’ve ever seen for these indications. It has a high margin of safety and the protocols provide a diverse population good opportunity for successful treatments.

Any thoughts on how the technology should progress?I think the Advatx was designed with a lot of forethought and, as such, there really is not much I would change. As a platform, it blends so well with other non light-based therapies that any of them would be a good addition to provide an all-in-one solution to aging skin.
