GENERALITES
• Prolifération monoclonale de petits lymphocytes matures par la MO
• Accumulation dans : MO + sang + ganglions + rate
• Fréquente +++
• Evolue lentement
• Décès cause infectieuses +++
• Transformation en lymphome agressif =Syndrome de Richter)
DECOUVERTE / DIAGNOSTIC
• Hyperlymphocytose > 4G/L
• Anémie
• Thrombopénie
• Palpation de ganglions anormaux
• Asthénie (Sd anémique), Sd hémorragique (rare)
• Infections à répétition
Confirmer le diagnostic
- Visualisation Lymphocytes B matures petite taille
- Immunophénotypage = score de Matutes (>4/5)
- Caryotype + biologie moléculaire = valeur pronostique
Immunophénotypage des lymphocytes circulants
EXAMEN CLINIQUE
 Déterminer le nombre d’airesganglionnaires atteintes
Aires ganglionnaires :
1 : cervicales2 : axillaires3 : inguinales4 : splénomégalie5 : hépatomégalie
EXAMENS COMPLEMENTAIRES
2/ Electrophorèse des protéines plasmatiques
 Hypogammaglobulinémie (infections)
1/ NFP
 Anémie ?
Thrombopénie ? Insuffisance médullaire
Neutropénie ?
Importance de la lymphocytose
3/ Scanner corps entier
 Rechercher des ganglions profonds
CLASSIFICATION DE BINET

A <3 non non Excellente
B >3 non non Bonne
C Peu importe <10 g/dL < 100 G/L Moyenne
Stades A = surveillance
Stades B = traitement à discuter (mais souvent oui)
Stades C = traitement
TRAITEMENTS
1/ Immunothérapie = Anticorps anti-CD20
- Rituximab MABTHERARéaction liée à la perfusion : désaturation, fièvre, frissons…

DECOUVERTE / CLINIQUE
• Syndrome tumoral : adénopathies, splénomégalie, hépatomégalie
• Altération de l’état général (AEG) = asthénie + amaigrissement + anorexie
• Symptômes “B” :
- sueurs nocturnes profuses
- amaigrissement
- fièvre vespérale
- +/- prurit
DIAGNOSTIC
BIOPSIE GANGLIONNAIRE
• Sous échographie• Sous scanner• Chirurgicale sous AG
• Examen anatomopathologique morphologie des lymphocytes
• Immunophénotypage CD exprimés par les lymphocytes pour déterminer le type de lymphome
BILAN D’EXTENSION
• Scanner corps entier (TAP)
• TEP-scanner hypermétabolisme des ganglions lymphomateux
• BOM
• Prise de sang : immunophénotypage lymphocytes circulants
TEP scanner
STADES DU LYMPHOME
Atteinte sus-diaphragmatique Stade I = 1 aire ganglionnaireStade II = >1 aire ganglionnaire
Atteinte sus et sous diaphragmatique
Atteinte extra ganglinonnaire
Pronostic : Stades I > Stades II > Stades III > Stades IV
Classification de Ann Arbor
Différence LH / LNH
Lymphome de Hodgkin Lymphome non hodgkinien
• 20-30 ans / 65 ans
• Symptômes B +++
• Bon pronostic
• Traitements :
- Forme localisées =
chimio + radiothérapie
- Forme diffuses =
