� Diagnostic de maladie coronaire chez les patients symptomatiques à probabilité intermédiaire ou haute de coronaropathie avant ou après test fonctionnel
� Diagnostic d’une douleur thoracique aigue chez des patients à probabilité intermédiaire avec ECG normal et troponine négative
� Détection d’une ischémie myocardique silencieuse chez patients asymptomatiques à probabilité intermédiaire ou haute de coronaropathie
� Bilan étiologique devant des troubles du rythme ventriculaire ou supraventriculaire
� Evaluation du risque opératoire avant chirurgie lourde non cardiaque
� Diagnostic étiologique d’une insuffisance cardiaque des patients à probabilité intermédiaire de coronaropathie, avec ou sans douleur thoracique
� Evaluation pronostique chez les patients
� A probabilité élevé de coronaropathie
� Coronarien connu � - symptomatique ou asymptomatique
� - avec ou sans coronarographie préalable
- avec ou sans revascularisation myocardique préalable (> 2 ans si asymptomatiques)
- avec aggravation récente de symptômes
- Avec épreuve d’effort impossible, non diagnostique, litigieuse ou de pronostic intermédiaire
� Recherche d’ischémie et viabilité résiduelle après infarctus du myocarde en particulier non revascularisé ou non coronarographié et dans les cardiopathies ischémiques pluritronculaires à FEVG altérée
� Aide à la décision thérapeutique chez les patients coronarographiés pour décision de l’indication et de la technique de revascularisation, pour la détermination du / des territoires(s) ischémique(s) et l’evaluation d’une sténose de sévérité intermédiaire

� Gating myocardial perfusion imaging allows the evaluation of :� Myocardial ischemia
� Myocardial cell integrity, thus viability� Left ventricular function and volumes
� Possibly contractile reserve
� Tracer uptake > 0.50 by quantitative analysis in 50% or more of left ventricle is the usual threshold for defining viability
� In these conditions, MPI is able to predict improvement in regional and global LV function with NPV = 80±5 % and PPV = 70±5 %
� “Viable” patients on MPI do benefit from myocardial revascularisation (functional status, survival), while revascularisation has no or harmful effect in “non-viable” patients.

Myocardial viability and
heart failure
Transplantation cardiaque : excellent résultats, mais le nombre de donneurs est limité.
Traitement médical (ACE, BB, ) : reste sub-optimal.
Revascularisation: peut améliorer la fonction cardiaque, les symptômes et le pronostic s’il y a du myocarde viable, mais la chirurgie est associée à une morbidité et une mortalité plus élevée chez ces patients.

avec coronaropathie suspectée ou avérée, et ceci dans de nombreuses
situations cliniques et de nombreux sous-groupes de patients
(revascularisation par stenting ou pontages aorto-coronariens).
� Elle apporte une information additive et indépendante de celle de la
clinique, l’ECG, l ’EE et la coronarographie
� Elle est un outil de grande valeur pour l ’aide à la décision thérapeutique en
pratique quotidienne notamment en post-infarctus en cas de viabilité.
� La scintigraphie myocardique de perfusion apporte toutes les informations
nécessaires à la prise en charge diagnostique et pronostique des patients
