- On appelle méthode de Monte-Carlo toute méthode visant à calculer une valeur numérique et utilisant des procédés aléatoires, c. à. d. des techniques probabilistes.
- Développée lors de la IId guerre mondiale par von Neumann et Ulam pour mettre au point la bombe atomique.
- Pourquoi Monte-Carlo ? Car les meilleures suites de nombres aléatoires sont données par la roulette des casinos…!
Simulation de Monte-Carlo adaptée à la microbiologie et la pharmacologie clinique – Principe
1. Inventer des CMI et des ASC aléatoires à l’aide de fonctions de probabilité.
2. Des milliers d’estimations ponctuelles peuvent ainsi être faites. Leur probabilité en est déterminée.
3. Tout les rapports ASC/CMI probables sont générés et la probabilité de les atteindre est calculée.

4. Using available PK/PD data, Monte Carlo simulations are performed and a tentative breakpoint calculated
”Minimum requirement for S-category” is that the high MIC value of the wild type MIC-
distribution is consistent with the MIC derived from the PK/PD index needed for optimal
efficacy based on free drug”.
EUCAST philosophy
Breakpoint may not overpass the Monte carlo simulation breakpoint
Lower breakpoint may as close as possible of the epidemiological cutt-off
Tentative breakpoints are checked against target species wild type MIC distributions to avoid splitting the wild type

Trois propositions sont faites: • Soit le concept d’activité sur les MRSA est faux auquel cas
le breakpoint est 0,5 mg/L (Si on compare les CMI de l'oxacilline (breakpoint 2mg/L
pour oxacilline correspond à à 0, 5mg/L pour la ceftaroline) . • Soit sur la base des Pk /PD on choisit 1 mg/L, dans ce cas
on coupe la population MRSA en plein milieu de la distribution des CMI
• Soit on monte jusque 2 mg/L pour englober la population MRSA homogène de 0, 5 à 2mg/L. On sait que les échecs cliniques surviennent à 4 mg/L. Le laboratoire n’a pas de données suffisantes pour affirmer que l’activité clinique de la ceftaroline est bonne sur les MRSA.

Inoculum
• La méthode nécessite d’obtenir un inoculum dont la turbidité est identique à l’étalon 0,5 de la gamme de McFarland*.
* Pour E. coli. cela correspond approximativement à 1-2 x108 UFC/mL
