State of the Art
� Peu de données récentes dans la littérature
� Inversion de la démarche par rapport aux années ‘’70’’
� La recherche a favorisé la prévention de la toxicité (pionniers du TDM)
au début, elle s’est ensuite orientée depuis une trentaine d’années vers
l’optimisation de l’efficacité des ATBs.
� De plus, peu de nouvelles molécules, mais dans presque toutes les familles
les molécules les plus récentes présentent souvent une meilleure tolérance.
� Enfin, l’approfondissement des propriétés PK/PD des ATBs à conduit à une
utilisation plus rationnelle des ATBs avec, notamment une réduction de la durée
des TRTs.

� Neurotoxicity is the most reported serious adverse effect of β-lactams.
� Benzylpenicillin, cefepime, ceftazidime and imipenem are considered to be
the high-risk b-lactams for neurotoxicity.
� Renal impairment, excess doses and/or concentrations, age and a prior
history of neurological disorders are known to be predisposing factors
� Concentration monitoring in high-risk patients could be a potentially useful
tool to avoid such effects, although adverse effects are considered rare.

Modalités de prévention
• Aminoglycosides should be given as a large single dose for a successful therapeutic outcome
– Multiple small doses will lead to treatment failure and likely to lead to renal toxicity
• Aminoglycosides are toxic drugs and require monitoring
– Avoid use in renal failure but safe in liver failure
– Avoid concomitant use with other renal toxic drugs
– Check renal clearance, frequency according to renal function.

Clinical trial data were evaluated for anemia, thrombocytopenia, and neutropenia.
Thrombocytopenia and a slight increased risk for anemia were evident at ≥2 weeks of
linezolid treatment. Hematologic abnormalities were consistent with mild, reversible,
duration-dependent myelosuppression.

developing QT prolongation and torsades de pointes, including older age, female
gender, diabetes, renal insufficiency, an underlying cardiovascular condition (eg,
hypertension, left ventricular hypertrophy, paroxysmal atrial tachyarrhythmias,
heart failure, valvular heart disease, coronary artery disease), polypharmacy,
and a combination of these and other factors.

treatment, and monitoring high-risk patients with regular ECGs may help minimize

proteins creating cytochrome complexes are disturbed in their structure leading
to permanent electron leakage and OS.
o (b) The complexes of FQ with proteins and cations are so stabile that they exist
in the cells by many years disturbing energy production and epigenetics.
o (c) Epigenetic changes in gene regulation become persistent many years of FQ
application even in the case of lack of FQ in the cell.

Possible effective treatment of FQAD are proposed:
(a) reduction of the oxidative stress, (b) restoring reduced mitochondrion potential
ΔΨm, (c) supplementation of uni- and bivalent cations that are chelated by FQs and
probably ineffectively transported to the cell (caution must be paid to Fe and Cu
because they may generate Fenton reaction), (d) stimulating the mitochondrial
proliferation, (e) removing FQs permanently accumulated in the cells (if this
phenomenon takes place), and (f) regulating the disturbed gene expression and
enzyme activity.

� Ethambutol : la plupart de ses effets indésirables sont dose et temps –dépendants et plus fréquent à
des doses > 15 mg/kg
� Pyrazinamide le plus hépatotoxique.Adaptation de la dose au poids du patient. Peu de risque si la
dose maximale ne dépasse pas 35 mg • kg-1 • j-. Chez des patients ayant une hepatite induite par la
PRZ, il faut, arrêter momentanément la PRZ ou la remplacer
� La Rifampicine interfère avec l’uptake de la bilirubine de façon concentation-dépendante .
� L’isoniazide: La neuropathie périphérique (20% des patients) et/ou l’hépatoxicité sont dose-
dépendante et peu fréquentes en dessous de 5mg/Kg/j. Le polymorphisme d’acétylation joue un
rôle utile dans le choix de la posologie et la prévention des Eis .
� Les EI des antiBK sont globalement posologies-dépendants, d’autres facteurs jouent un rôle tels
que, l’âge, l’état nutritionnel, la fonctionnalité des émonctoires, l’alcoolisme ou l’existence d’une
co-infection (VIH).
