Critères diagnostiques du SHR• Critères majeurs :1. Réduction du débit de filtration glomérulaire avec une

créatininémie > 1,5 mg/dl ou clairance de la créatinine des 24h < 40 ml/min en dehors de l’administration de diurétiques.

2. Absence d’état de choc, d’infection bactérienne évolutive, de pertes liquidiennes, ou d’administration de médicaments néphrotoxiques.

3. Absence d’amélioration de la fonction rénale après arrêt de la prise de diurétiques ou d’administration d’une expansion volémique.

4. Absence de protéinurie > 500 mg/j ou de manifestations échographiques en faveur d’une uropathie obstructive ou d’une maladie du parenchyme rénal en faveur d’une insuffisance rénale organique.

• Critères mineurs : 1. Diurèse < 500 mL/j

2. Excrétion sodée urinaire < 10 mmol/L

3. Osmolalité urinaire > osmolalité plasmatique

4. Absence d’hématurie définie par un nombre de globules rouges urinaires < 50 sous champ microscopique à fort grossissement.

5. Hyponatrémie de dilution < 130 mmol/L

• SHR 1: classique, IRA de type fonctionnelle

réduction rapide la filtration glomérulaire rapide de la Créat > 2,5 mg/dl en < 15j; Oligoanurie

• SHR 2: Créat < 2,5 mg/dl
