Une patiente de 36 ans se présente au cabinet avec une 46 ayantperdu sa coiffe prothétique depuis plusieurs jours après des descel-lements à répétition. (photo 01)

L’examen clinique révèle une perforation carieuse du plancherobjectivée par une radio rétro-alvéolaire qui montre de plus unelésion asymptomatique à l’apex de la racine distale. (photo 02)

Nous prenons ensemble la décision d’extraction et le remplacementpar une prothèse implanto-portée.

L’anatomie osseuse ainsi que la qualité parodontale des tissus envi-ronnants permet d’envisager une extraction-implantation immé-diate dans le secteur inter radiculaire.

L’avulsion est pratiquée après séparation des racines. (photo 03)

Le site opératoire post-interventionnel ne montre aucune lésionosseuse vestibulaire iatrogène.

L’absence de lambeau préserve l’architecture gingivale et maintientla vascularisation alvéolaire.

Le kyste en regard de l’apex était radiculo-adhérent et il n’a pas éténécessaire de pratiquer un curetage .(photo 04)

Les éléments étant favorables, la décision d’implantation est vali-dée.

Le choix de l’implant se porte sur un implant Champions révolution,3,5 mm de diamètre et 12mm de long.

Il est mis en place après une préparation classique MIMI (utilisationde forets coniques 3 faces et condenseurs osseux manuels de 2,4mm et 3,0 mm ).(photos 05 et 06)

Le couple de serrage de 35 NM permet une bonne stabilité pri-maire. On utilise ensuite un clip gingivale de protection et deuxsutures mesiale et distale (la stabilisation du caillot sanguin favo-rise la cicatrisation osseuse ). (photo 07)

Il n’y a pas d’utilisation de matériaux de comblement osseux.

Nous prenons la décision de temporiser et de ne pas utiliser decouronne provisoire.

A trois mois la stabilité est satisfaisante avec une régénérationosseuse et un comblement alvéolaire en bonne voie. (photo 08)

A cinq mois la prothèse est mise en place.

On observe une perte osseuse vestibulaire mais le niveau gingivalcervical a été en grande partie préservé permettant ainsi unebonne intégration esthétique. (photo 09)
