• Volume érythrocytaire = VST x Ht (mL de GR à Ht=100%)
• PSA = VST(Htinitial – Htseuil)
• BTP = PSCe - PSA
Transfusion
Autologue Programmée (TAP)

• La transfusion est adaptée au débit du saignement observé, de façon à maintenir [Hb] > seuil
• Il est recommandé de surveiller régulièrement l’hématocrite ou, mieux, l’hémoglobine au cours d’une chirurgie hémorragique.

Préop : l’EPO humaine recombinante
• Objectif : volume érythrocytaire
• Indication : chirurgie à risque hémorragique élevé et 10<Hb<13g/dL
• CI : HTA non contrôlée Atcd thrombose arterielle, sténose carotidienne, angor instable
Hb>15, Ht>50%
• Délai >3 semaines
• Modalités : protocole Binocrit (ordo d’exception)
– 1 injection SC à 600UI/kg
• J-21
• J-14 – NFS de contrôle : arrêt si Hb>15
• J-7
• Le matin de l’opération si Hb<15g/dL
– Associé à un supplémentation martiale : Tardyferon 200mg/j per os
• Délai < 3 semaines :
– 1inj/j à 300UI/kg de J-10 à J4
Perop : mesures générales
• Normothermie : prévention de la coagulopathie de dysfonction plaquettaire
• Profondeur de l’anesthésie : VO2
• Normovolémie
• Adapter le monitorage
Perop : RSPO
• « Transfusion autologue peropératoire après récupération de sang épanché » AFSSAPS 2002
• Indications : pertes > 15% volémie théorique calculée
• CI : sepsis, chirurgie carcinologique, metallose(reprise)
• Utilisation postopératoire immédiate possible (6h)
• “Tranexamic acid was effective in reducing the need for blood transfusions while not increasing the risk of complications, including thromboembolic events and renal failure. Thus our data provide incremental evidence of the potential effectiveness and safety of tranexamic acid in patients requiring orthopedic surgery.”

Postop : Fer
• IV :
– Venofer (saccharose ferrique) 300mg dans NaCl iso(250cc) à J0 et J3
– Ferinject (carboxymaltose ferrique) 1000mg dans NaCl(250cc) sur 15min à J0
• Per os (Tardyferon 100 à 300mg/j) pendant 3 à 6 semaines
